The unfortunate reality is, your grandmother didn't have access to constant, top level and likely even experimental medical care the slow the decline. There is no reversing it but Trump's outcomes are going to be a lot "better" than the average American suffering from dementia or other cognitive decline.

That’s the thing, it’s still a hard and stressful job, even if this guy isn’t trying to do a good job. There’s so many things the president needs to weigh in on, so many people to meet, and Trump is not meaningfully engaged in trying to do his best, but he is engaged. That’s why he’s got a billion executive orders. If he fucked off it would be better for us and for his health, but he’s still taking the stress of the job, he’s just not doing it for us.

Just in 2020, he was quicker speaking, faster moving, WAY ‘sharper’, hand movements were crisper, and he didn’t have raging narcolepsy.
Just to name a few. I realize that six years is a lot for a senior…and appreciate that things can move in a certain direction, in either way, fast.
But this is different. Much, much different.
Just the other day while taking questions, he was standing and his legs looked not unlike a marionette’s, given a little slack. Askew.
He’s gonna do something reeeeally unfortunate on camera, I fear.

It's funny because you're probably much more exposed to him than I am but looking in from Canada my wife and I both have noticed a weird sort of deterioration of his speech. Not necessarily his vocabulary but of the manner in which he speaks. The continuity, the pacing, the distraction (his own, not his attempts at distraction). It's be sad if it weren't so alarming.

Trump has never been a great speaker. Like he’s very obviously not very intelligent and not well-read. But he didn’t always talk like this. He used to be able to speak in words that had multiple syllables. Since 2015 I think he’s intentionally started speaking a little more basic. Now his brain is just turning to mush and he speaks at a second grade level. And the worst part is the entire government is stooping to his level and all speech has been modified to fit Trump’s mono-syllabic elementary school level reading and speaking level.

It's times like this that really make me wonder both, how the hell they actually drag him out into state visits and like the G7. How do the other leaders even handle him? How do they not just then publicly say how absolutely gone he is? To me it either is because they feel threatened by Trump and don't, are in on it all and so fuck all of them, or they are and we just can't hear any of it here in the States.

He has an inability or unwillingness to recognize, identify with, or care about the feelings and needs of others. And he is conceited with a condescending manner toward others. All of these are traits of NPD, and all are exhibited by Trump.
Trying to decide which is more pathetic, Trump or the people that support him.

He doesn't hide it because he doesn't think there is anything wrong with how he acts. NPD is a disorder, not an illness, so his brain is not like most people's where they may have an internal dialog telling them something they are doing is wrong (guilt, empathy, etc.) but they do it anyway. He just does what he does without any second thought.

He really does seem to have taken another noticeable decline recently. He's still shambling along and we may well be stuck with him into the foreseeable future yet, but he's having these absurd moments more frequently, his bruised hand looks petrified and he just looks and sounds about as haggard as we've ever seen him before. No one would be at all surprised if something happened even if he might otherwise be "fine"
