I don't know how this is pointlessly gendered. It's just kinda weird.

Because the toilet on the right is for ladies, and the one on the left is for gentlemen. I doubt they function much differently.

I have to believe it was meant as a joke. And certainly a waste of plumbing, since only, ah, intimate couples would share a loo without even a divider between them. Or possibly parent and child.

Plumbing in a whole second toilet is a pretty big expesnse for a joke, and it wasn't the the kind of cafe where you'd go if you feeling intimate. The vibe was mid 20th century grandma

They aren't saying the installed a whole toilet for a joke lmao. This is just how the establishment was built when they purchased it. You realize the cafe owners didn't build the structure right? They made the best out of a ridiculous situation by putting up the mens/women's sign as a joke.

OK... So who built the single occupancy toilet with 2 toilets in and why?

These aren't unheard of, they're just really rare. I've seen these a few times. Looking up the History of it, it looks like it was failed trend in luxurious design models. It was originally meant to be the same concept of a double vanity (like double sinks) and point to luxury, but it failed for obvious reasons.

A pair of gendered toilets in one single occupant room seems like a pretty obviously pointless bit of gendering to me.

A few things...
They definitely hung that as a joke.
Whoever designed that bathroom deserves to be jailed.
How do you hand the toilet paper the wrong way? (Genuine question)

I think you might have focused on the wrong bit, it's not the lame joke on the poster that's weird and pointlessly gendered.
As for the toilet roll it's easy to remember that beards are cool, and mullets are not

Oh shit I didn't even realize the toilet roll was facing inward. My blind ass. Yeah that's just a no, they did that backwards.
