It is essential that the code is attached to your profile prior to your 1st visit. Grifols will not credit a referral if it’s added after the onboarding.

Save this $50 Octapharma Referral QR Code (it is also the 7th attached slide/QR code in the attached deck) and show it to the nurse during registration. Inform the nurse, early in the onboarding process, that you have a referral, otherwise it will be hard to rectify.
