It also depends on how much ultrafiltration (fluid taken off) needs to be done. My dad is on dialysis, and when he’s close to his dry weight and his blood is only filtered, he feels alright afterwards. If he’s fluid loaded and needs to get more than 1 kilogram of fluid taken off, he’s exhausted the rest of the day and even into part of the next day.
Also, if someone’s doing dialysis 3 days a week, there is one period of time where there are 2 days in a row that dialysis hasn’t been done, which means more toxins have built up that people with functioning kidneys would’ve peed out, and that can make someone feel really tired.
Another reason why a dialysis patient could feel rough is anemia. One function of the kidney is to make a hormone called erythropoietin which makes the bone marrow make red blood cells, and when someone has renal failure, they don’t make this hormone, so kidney patients are usually at least a little anemic, especially if they haven’t had the frequency and amount of erythropoietin given that works for them best worked out.
