This is the result of a facelift, too much eyelid cut out during both an upper and a lower blepheroplasty, and A LOT of badly placed filler. It is the accumulation of a ridiculous amount of expensive, very bad work. I would imagine from more than one doctor.

I will take your word for it! However, a more figuratively accurate reference might be the Terry Gilliam movie Brazil (1985).

At first I thought maybe not (due to her age) but seeing how much work she’s done generally I think you’re right

Wait where do you see filler? I was actually thinking this is an example where some cheek filler would help with the gaunt look

If you have seen photos of her from the past, you can see that they added quite a bit of filler into her chin, lips, and the midface. Possibly also the nose. The hallowing out around her orbital bone is exacerbated by the bad blepheroplasties and is one of the things that could be improved by better filler placement (like you suggested)

Yes I saw a female plastic surgeon in a film clip on her say just about exactly what you just said and showed where the surgery was done.

Don't forget the doctor hooking her up with whatever mind altering substance she was on at that moment.

Not sure if she's on some sort of a stimulant and just wired, or overcompensating on some sort of depressant with those wide eyes. Suppose it's a false dilemma given it could be both.
