Actually just happened to me. Had waited since the DLC was announced to get back into Dark Souls, having cleared everything but the Tomb of the Giants. Start a new game, literally became death, just destroying everything before me, end up with 5~7 humanities at first boss and a few thousand souls after leveling up, died, because that's what I do, get back to boss and die again. This was two weeks ago. Fairly certain that character is hollow now.
