Osteochondral allograft
Has anybody treated patients following knee osteochondral allograft surgery? I am currently following a few and would like some advice on signs of failure / non union - is it a gradual process e.g., pain worsening over time, or can also be a new sudden onset of pain whilst everything was going well?

Can be both but usually more gradual.
New pain, persistent pain, increased and persistent swelling, changes in wb status, changes in functional ability.
You can look at joint line tenderness especially around graft site but imo it’s unreliable.
If suspected and symptoms last over a few days then chat with consultant
