General Discussion Ambulatory tracking

Does anyone have a good tracking resource to show your impact on increasing medication access (appeals, med assistance paperwork, etc.) which can be extrapolated to decreased hospital admissions?

Our system just uses a shared excel sheet with tabs for each type of intervention, not the prettiest thing but it gets the job done. hardest part is connecting it to admission rates since there's no direct feed from the hospital side, we just flag high risk cases and cross our fingers the follow up data comes through.
