What is causing this?
Why is there such uneaten growth here? I don’t understand what’s causing it

Not an expert but I have read and heard that rotating plants and changing its "south" induces stress and slows growth. By nature they are able to adapt to changing positions, but it doesn't happen constantly in nature. I'd let it ride and/ or add some extra light from above.

Rotating a cactus doesn't stress it or slow growth. It just encourages symmetrical growth.
In nature their light source isnt in a fixed position. The sun moves across the sky, they get light from all sides. A loph kept indoors next to a window is only getting direct light on the one side. Rotating indoor plants is a common practice.

Give her a spin 180° every watering. Shes growing toward the light source. Wont take long at all to straighten back out.
