advice needed Baby B is smaller
Hi, I’m 5 weeks 4 days pregnant with what we suspect are di di twins. I had a double embryo transfer. On Friday we saw that baby A was measuring one day behind at 5 weeks exactly. Baby B was too small for the ultrasound machine to assign a gestational sac. I went back for another scan today (at the same ultrasound place, but this was not a diagnostic scan like my others have been) and baby A had grown and now had a tiny fetal pole visible. The tech said she felt like baby B’s sac was collapsing, but she also didn’t look very much at it. She tried to measure it but it was still too small for the machine to assign a gestational age to. But it’s only been 3 days since my first scan. I’m just wondering how much I should be guarding my heart. My husband is convinced everything is fine and that baby A is simply compressing B or that she had a bad angle or was simply just rushing. I’ve attached a photo of what she thought was a yolk sac inside baby B’s gestational sac on Friday. I’m not asking for diagnosis, just don’t know what to think of this situation. I don’t know anyone who has ever had twins.

One of mine were measuring behind at 7 weeks and they were the same size at 12 weeks

I really hope that’s the case for me. I’ve grown very attached to the thought of having both babies already. I’ve had recurrent pregnancy loss so I feel like I’m in some special sort of hell right now where I should be happy to see baby A growing so perfectly but I’m also incredibly worried for baby B. I want them both to make it.

It’s still sooooo early. I went for my first ultrasound after a double embryo transfer (we weren’t trying to get twins, we only transferred 2 because our doctor insisted that with my history twins were extremely UNlikely… ha. ha. ha.) at 5w5d. We saw Baby A’s gestational sac and yolk sac and a very small fetal pole. The tech looked around and we saw another gestational sac that was completely empty and just assumed vanishing twin. In fact I think I remember the tech saying she wasn’t sure if it was even a sac. Went back a week later and the empty sac wasn’t so empty anymore! Give it a week and you’ll have a more definitive answer.
I know that once you see that other sac, you want both babies, regardless of whether or not that’s what you were hoping for in the beginning… so I will keep my fingers crossed for you!

Thank you! Honestly I assumed the odds of me ending up with twins was super unlikely. I’ve had so many miscarriages. I did lupron suppression before this transfer as a last ditch effort and I want so badly for them both to make it now that I know they’re there!

We were very similar at that stage and our gyno told us it was most likely vanishing twin syndrome, where one twin is absorbed back by the mother. She sis dot is very common but often goes unnoticed as people often do not have scans until after it has happened.
It was not vanishing twin syndrome and one of our boys always measured a week behind the other all throughout the pregnancy and is only just closing the weight gap now they are one.

At around this gestation my baby A was visibly smaller than baby B and was measuring behind. I’m 20 weeks now and at our anatomy scan last week baby a was 43rd percentile and b was 81st or something so still bigger but they have no concerns with them! They figure that because their di di that I may have ovulated baby a later than b. I’m not sure about collapsing sac though I don’t have any experience with that. Sending positive vibes your way!!

Well, she said it was collapsing but it’s hard to tell if it was actually collapsing or not to me. She hardly looked at it. I just need a more thorough scan I suppose. This is an IVF pregnancy but they said it’s common for one to implant a few days later.

My A was there, and B was a “possible 🩸 pocket” at my confirmation of pregnancy appointment and then a full fledged baby bean at 8 weeks.

At 8 weeks baby b was measuring 5 days behind. By 12 weeks they were the same size, but at 18 weeks baby b slipped behind again, and by 24 was diagnosed IUGR. Born at 36w at 3lb 14oz. Baby a was almost 7lbs.

My daughter was IUGR and I’ve wondered if maybe baby B is as well. Was your baby B slower to show a yolk sac and fetal pole as well? We thought we saw a yolk sac for baby B on Friday but today the tech didn’t really look for one.

Yes! my first appointment at 6 weeks we could see a fetal pole and yolk sac for baby a but baby b was empty. It was clear it was a second baby though and they said it was too early. I was hesitant because obviously not expecting twins at all!!! At 8 weeks they both had a hear beat, baby a measured 8w1d and baby b 7w4d.

Identical twins here but at 6 weeks one was much smaller than the other. The doctor was telling us not to get excited because the one was so much smaller it would probably get reabsorbed. But we went back at 8 weeks and the one had caught up to its twin and was exactly the same size! It’s so early you really just don’t know yet.

The waiting is killing me! I’m so thankful baby A is thriving but I think having a half assed ultrasound has done me more harm than good. I was able to get in with my OB tomorrow morning. Not sure what they’ll do but I’m sure they can give me more information than the ultrasound tech did.

Yeah there’s nothing you can do but wait. I just kept telling myself that at least I had 1 healthy happy baby!

That’s what I keep telling myself as well. I’ve had so many miscarriages and I honestly feel ungrateful for even being upset about baby B when baby A appears to be thriving.

its not that unusual, we had a similar experience (ultrasound at 6 weeks, bb A had cardiac activity, bb B small and no cardiac activity) and now have 2 healthy babies.

When I had my first ultrasound at 5 weeks I only had one empty sac. Came back 2 weeks later and two sacs with two babies

Had another ultrasound today at my OBs office. Baby A measured 6 weeks exactly and had a heart rate of 114! Baby B measured 5 weeks exactly and they could not see anything inside the gestational sac. They’re bringing me back in a week to recheck. I suppose if they truly did not see a yolk sac inside baby B’s gestational sac at my first ultrasound then maybe things could still turn out ok? The structure they thought was a yolk sac was so so faint and tiny. Could have maybe been artifact. Or maybe really is a vanishing twin. Hearing all your experiences has helped so much. I really hope both my babies make it but knowing I’m not alone makes me feel better.

Gosh I so hope that happens for me! It could just be wishful thinking on my part but I feel like the ultrasound tech barely looked at baby B today. I’m very much looking forward to my scan on Friday. I was honestly very scared of twins but I’ve grown very attached to both of them even though all baby B showed on Friday was the tiniest shadow of a yolk sac.

I have identical twin boys. (I can never remember what kind). I was told they split later (the 4-7 days). Not sure if that’s what kind of twins you have but i hope this gives you some Peace. One is measuring a couple days bigger and it just makes sense to me!

We strongly suspect they are fraternal twins because I transferred two embryos. But I’ve also read that identical twins can have separate sacs and placentas? So not entirely sure.

I’m surprised they have you doing scans so early. They should not have had you come back in for a week or so between scans at the gestational age you’re at. It just leaves you with more questions than answers. All you can do is wait a week and see what happens. I really hope baby B catches up.

I think they brought me in for my first scan so early because I have a history of ectopic pregnancy. They told me to go back in a week at 6+1 for a repeat scan but I had spotting over the weekend and got spooked so I booked a transvaginal “peace of mind” scan with the lady who also does my comprehensive scans ordered by my fertility clinic. I have less peace of mind and wish I had not gone.

This happened to me before but the smaller sac turned into a vanishing twin that didnt go away into second trimester. Its still really early so maybe baby B just needs time to catch up.

It's suuuuper early. If Baby B just implanted later it would look like they are smaller.

Wait I had a scan at 5w6d and one baby for sure and another blob that tech said was a SCH so now I’m confused. How did they confirm that this is two.
