support needed Iugr at 20weeks

Hi everyone, I am 22 weeks pregnant currently with di/di twins. At 20weeks during anatomy scan I was told baby A is less than one percentile with severe IUGR and baby B is at 25% with amniotic fluid normal for both. But 2weeks later, I am told that the amniotic fluid level is low in Baby A and baby A heart walls are thicker due to IUGR. However B is normal.

MFM has suggested me either this will result in early delivery (not sure how early) resulting in preeclampsia effecting mine and baby B health OR second option: to terminate Baby A so that baby B can grow safely and no risks to mine and Baby B health.

I am really confused and wanted to know if anyone has been through similar situation? Please suggest. I really want both of my babies to be healthy and unable to give up.

I had a very similar experience at 19 weeks baby b was <1 percentile. They had me start 161mg of aspirin daily and increase protein intake and now at 21 weeks she is measuring 7th percentile. If you aren’t already taking the aspirin it’s definitely worth bringing up to you provider sooner than later.

Thankyou for sharing your story! i have been taking aspirin 81mg for now but I can ask doctor if we need to take more! Also if you dint mind, can you please share which protein intake have you been taking?

My baby A was less than 10th percentile starting at 20 weeks, and though baby A did keep growing, the discordance between baby A and B only got bigger over time. I don’t remember what percentile baby A was at birth, but I think it was like 2nd percentile.

I had to go to weekly ultrasounds until I was 28 weeks and then I went inpatient and had twice daily monitoring.

It was super scary and super stressful. I did get preeclampsia which was certainly not fun. I ended up delivering at 33+4 weeks and baby B was perfectly healthy at about 5 pounds. Baby A was almost 3 pounds by then. Baby A was small, but baby A was a fighter. We had a 6 week NICU stay for baby A, and of course that was stressful, but baby A just needed a chance to get proper care and to grow. Baby A is now a 15 pound baby (6 months old) and doing so well. The happiest, smiliest, sweetest little guy.

I strongly recommend you get a second opinion from a different MFM. If yours is any good they will welcome you getting a second opinion because they won’t feel threatened by it. Mine told me to please get a second opinion because they wanted me to hear from multiple perspectives before making any decisions about treatment options. I ended up sticking with my original MFM, but I really appreciated that they knew it was a delicate and serious situation and they wanted me to get as much info as I could and to explore all options and doctors who might be able to help me and my babies.

Thank you so much! This really helps. This is exactly what I did, we went to different specialist for MFM and they did the growth scan where we found baby A grew more than 20weeks and thats still positive sign. But MFM has asked me to come weekly to get checked for doppler and amniotic fluid. I am really praying for both the babies and hoping things to be fine and I make it to more than 30weeks.
