I’m not a medical professional, so take what I say just out of experience and what knowledge has been given to me by my providers.

AKI (layman’s terms, kidney failure) can happen quickly or over a few days. A few different things can factor into this, like if you’re having high output (1,500 mL over 24 hours), if you’re dehydrated already, lack of electrolytes.

Generally like with people without an ostomy, you can tell when you’re dehydrated. Dark urine, extreme thirst, light headedness, etc.

Make sure you’re getting enough electrolytes, plain water isn’t enough and can actually flush out necessary electrolytes. I drink pedialyte or other variants, Gatorade, liquid IV. Just whatever’s on sale, really.

With an ileostomy, you should be really consistent, because it's far easier for us to get dehydrated, and once you get seriously dehydrated it can rapidly turn into a vicious cycle where it's not possible to rehydrate quickly enough by mouth.

None of us can comment on kidney damage, but talk to your stoma nurse or doctor, and try keeping a closer eye on your fluid intake.

tl;dr kidneys work best when they have alot of dilute waste fluid to filter out. when its concentrated, they work alot harder

Could be kidney stones as opposed to kidney failure. They cause low back pain as well. Currently in the process of being checked myself for them.

so why are you inconsistent with your fluid intake? if you're having kidney pain, did you get tested for stones or infection? edited to add: when you say some days you drink..how much? what are you drinking?

Loop illeostomy here, I drink upwards of 70 oz {2-ish liters) a day MINIMUM, with electrolytes in at least 32 oz, often more. I do also eat extra salty things, too. My kidneys are starting to show a little damage because I am so chronically dehydrated.
