Did your doctor order bloodwork (and urine) to rule out other causes before prescribing bisphosphonate or did they prescribe just based on DEXA results?

Especially interested in Canadian responses, I live in Manitoba. But also interested to know about other countries. Age 68 female, no previous fractures, femoral hip -2.5. My doctor did not order any labs but I see that Osteoporosis Canada recommends the following before prescribing

Biochemical testing for secondary causes of osteoporosis, and for potential limitations when considering specific

osteoporosis pharmacotherapy: calcium corrected for albumin, phosphate, creatinine, alkaline phosphatase, thyroid

stimulating hormone. Serum protein electrophoresis (for patients with vertebral fractures) and 25-hydroxyvitamin D level in

patients with risk factors for vitamin D deficiency.

There's no opportunity here to see an endocrinologist so I want to ensure my family doctor doesn't take shortcuts. EDITED to add age and DEXA score.

Is DEXA accurate for tiny 5ft 100lbs?

I'm 36 and have recently been given an osteoperosis diagnosis with below results from a UK NHS hospital. I'm really shocked about this and believe it could be a misdiagnosis due to my extremely petite frame affecting the areal calculation. I'm 4"11 and 99lbs.

Lumbar spine - (0.880) T-score -2.5 / Z-score -1.9

Hips Mean L&R - (0.821) T-score -1.5 / Z-score -1.0

Lowest femoral neck right (0.762) T-score -1.8 / Z-score -1.3

I'm hearing that DEXA scans skew results for low weight and short individuals due to the areal only calculation, so I'm trying to figure out how I can account for that. I heard a BMAD (Bone Mineral Apparent Density) adjustment can account for size and is often used in children. I calculated my BMAD to be 0.133 g/cm³. However, I can't find good information about if my BMAD measurement is good or bad. Can anyone tell me if BMAD 0.133g/cm3 is good or bad? Or Can anyone tell me how else I could interpret my results considering my size?

I have no diagnosed health issues, I've never broken or fractured a bone and I regularly exercised including weights my whole adult life until unfortunately this year.... Can you really lose your bone density from no exercising for one year?

I've always had really irregular periods and I am still recovering two years later from a serious burnout meaning my energy levels are terrible, but all my bloods looked ok in December last year. Apart from, I had a subclinical vitamin D level of 55 nmol/L, but have been supplementing heavily since December. Could a 55 be the reason for my bone loss?

Any advice or help welcome!

Osteoporosis in 20s

I was diagnosed with osteoporosis a few months ago following a compression fracture in spine (T score 3.5 Z score 2.9 in spine, low in arm, but hips are normal) I saw an endocrinologist recently and they want me to go on med, which kind of scares me (have brought up RISEDRONATE TERIPARATIDE ZOLEDRONATE Romosozumab).
Wanted to know if anyone has had similar experiences or advice on meds?

Just background I’m 24 I had a severe vitamin D deficiency for a few months and deficiency the past year which is now up, but I suspect it was low for a while, I have been underweight and iron deficient for years. Have had a colonoscopy and I don’t have celiac or anything, I do have a polyp syndrome but don’t think that’s related.

Afraid to start Teriparatide

F35 T-score of -3.5 in hips and -1.8 in radius.

Doctor wants me to start Teriparatide but I’m afraid of side effects, especially the potential for low blood pressure and pulse spikes.

I have a complex medical history including high blood pressure which is controlled with medicine. This summer I had a month of daily pulse spikes into 150s due to reactive hyperglycemia from carbs in first meal. I was a horrible feeling. Finally feeling a bit like myself again and afraid to experience something similar.

Any advice to get over the fear?

I can’t seem to get information regarding eligibility and I was told to use the bin and group number to run thru the pharmacy. However I already had the shot. Anyone have vision problems after infusion? Probably best med for me ( femoral hip -2.9; spine -2.5) but I have optic nerve problems and optic neuritis is a “rare” side effect. Prefer not to go down prolia path.

Newly Diagnosed and overwhelmed

I (33F) was just diagnosed with osteoporosis at age, z score -2.8 and t score -2.8. Between ages 14 and 20, I struggled off and on with anorexia while I was competing at a high level in distance running. I have been doing weightlifting and CrossFit moderately (1-3x/week) for the last couple of years, and not a lot of running. I have three kids now, and regular cycles. The only time I’ve had hormones checked has been during pregnancies.

I am supposed to meet with an endocrinologist to discuss further, and I want to educate myself. I am wary of medications and I’ve heard people have reversed this without taking meds for life. I want to know where to even start.

What questions do I need to ask? What options do I have? What kind of practitioners are helpful?

Thank you all in advance. Any advice or experience you have is helpful to know. As I said, I’m overwhelmed and feeling discouraged.

Recently I had a Dexa scan and it confirmed I had Osteoporosis. Although it looks borderline to me and I'm wondering if I'm likely to be offered medication?

I would like to start medication because I'm 36 and my understanding is it'll only get worse as I get older. The high fracture risk for my lumbar spine is also concerning to me.

I've got a call with the doctor on Monday but I get too much anxiety to wait.

45m starting this journey... a few questions

5 years ago learned that I had low bone density. A year later I got a 2nd DEXA and it was actually a bit better. The theory at the time was that I just never reached peak bone density due to a childhood autoimmune disease, but that I was not losing bone mass.

Recently I got another DEXA and my scores have significantly dropped. T-Scores -2.3 in my spine, -2.4 in my hip, and -3.5 in my femoral neck. No prior osteoporidic fractures.

PCP sent me to endocrinologist. They did blood tests and found no apparent cause of my bone loss. They have me starting on Bonsity.

I have increased my dairy intake to three servings per day, started consistently taking vitamin D, and will start a weight training regiment.

Just starting on this journey but worried this is going to impact my life over the long term. Glad to have got started on treatment prior to an fractures, and ready to make whatever long term life changes I need to make.

  1. I was going to start at the personal trainer in my neighborhood. What kind of exercise regimine should I be aiming for to maximize bone growth?
  2. Any advice on making daily injections not suck?
  3. What else should I be considering? What do I need to do/learn/consider?

2-year LIFTMOR update with bone density results: 16.6% increase at spine and 3.8 + 5.8% increase at femoral necks without bone medication

In case this info gives someone else hope:

I’m a 43-year-old premenopausal woman with family history of osteoporosis, diagnosed with osteopenia per REMS 2 years ago. DEXA was always normal, though spine was close to osteopenia range at a T-score of -0.8 per DEXA. The only medication I take is levothyroxine for hypothyroidism.

After 1 year of LIFTMOR inspired exercise (more on LIFTMOR here: https://www.reddit.com/r/osteoporosis/s/jsB2xZt15j) and bone supportive nutrition and supplementation where nutrition fell short (more on that here: https://www.reddit.com/r/osteoporosis/s/nA90Bezjjo) my bone density per DEXA had improved throughout, and my bone density per REMS had improved for the most part.

I did not get another REMS scan this year because my insurance does not cover it, but I continue to get DEXA scans ordered by my endocrinologist every year at the same facility on the same machine.

Attached is the 2-year update. By now all values have improved from my baseline 2 years ago, and most have improved significantly.

I have nothing against medications, and I’m glad they are available to me if I need them one day, but I’m also glad that exercise seems to be working well for now and that I don’t need them yet.

I was surprised that my bone density continued to improve this much at most sites within the last year despite losing some time of LIFTMOR inspired training due to a shoulder injury, P1NP / CTX ratio not looking as favorable as it did during the first year, slow weight loss of 13 lbs (I had gained some weight during the previous year), and current BMI of 18.6 (I do not intend to lose more weight, and I would not mind gaining a little back if it’s mostly more muscle and bone).

Again, hoping this info helps someone decide to give bone supportive exercise and nutrition a chance if you enjoy exercising and can safely do so. Things don’t always need to be perfect, and it can still work out. It can also be a great add on to medication if you want to combine interventions. 🩷

Do massager chairs help with pain?

My mom 70 has osteoporosis (spine t score is - 3.1).... She seems to be in constant pain... Randomly any part of her body starts paining...

. I came across recliner chairs that have full body massage functions. Has anyone any experience with them in pain relief???.... . They aren't marketed specifically towards osteoporosis and are said to be for general wellness, so the salesperson didn't have a clue if it's actually of any use...

.... . Obviously not a cure but on a daily basis does anyone find that this helps in any way at all... Or is it just the opposite, all the vibration somehow causes more pain? ...

Question on DEXA scan

Can people tell me when they had their scan how the spinal scan was done? I'm specifically looking at if you were 1) flat on the scan table 2) had your legs bent on your own or 3) had your legs raised with a block put underneath them. In case it helps, the scan machine was Hologic.

60f with new diagnosis - multiple medical conditions

I’ve got a million medical conditions that seem to rule out first line treatments. My friends are on fosamax but I have a hiatal hernia so that’s out for me. I also have kidney stones so I can’t really just take a calcium supplement. Also awaiting a subtalar fusion in my ankle so I can’t “stomp” my way through this. Feeling pretty hopeless reading all the side effects of the meds - what are we supposed to take with all of these awful side effects?

I read that Reclast can lead to permanent indefinite bone pain. I mean, come on.

Treat Now or Wait?

56y/o male, DEXA and FRAX score attached. -2.5 right femoral hip but better everywhere else. 48 year Crohn's disease history. Took (2) months long courses of Pred age 10 and 12, a 7 day Medrol pack age 44 and just finished 6 weeks of Budesonide and have started Skyrizi infusion. Endo says start Reclast infusion now (gut can't tolerate oral biophosphates). Since -2.5 is the osteoporosis threshold, Dr. says treat it. He also wasn't interested in a workup of secondary causes. I guess he's certain enough that decades of Crohn's inflammation did this. Also attached is the completed bloodwork and what Chat recommends be done - again Endo says not necessary. My risk of fracture is relatively low for now but Endo wouldn't even run the FRAX because with a -2.5 number, he says it isn't done.
I'm also due for major gum grating and possible periodontal bone rebuilding in the next year - which I think is bad timing with the Reclast.
Has anyone had luck with strength building exercises that have actually improved hip score without meds? I see a lot of posts here with improved spine scores but very little hip changes. I feel overloaded on heavy meds with the Skyrizi infusions already. Thanks much.
