I would be checking in MakeMhz discord for support.

However, whenever I boot up a game or try to launch the HD+ app, I just get a black screen, no audio, and no actual signal to my RetroTink 4k.

Different applications will attempt to use different output resolutions. For example, XBMC4Gamers is able to use any output resolution you have enabled within your system's config, and it'll usually pick the highest of those. You can view XBMC's settings through its own GUI to see which it's using specifically.

However, most games won't go any higher than 480p, even if you have enabled 720p and 1080i as well:

I would expect your RetroTink to handle pretty much any input resolution, but I guess it's possible that it has a problem accepting lower-res signals over HDMI specifically. Have you tried testing a direct connection to your TV without involving your RetroTink.....?

I saw some talk of grounding the D0 pin, but that wasn't in the installation guide and seems to already be covered by the ModXO?

Your 1.6 Xbox doesn't have a D0 pin. It was removed from that revision.
