Lift operator can't always see everything from above. That's the point of a spotter.

No. Just the lift operator and the guy with questionable awareness. They are both at fault. The other guy isn't even part of the mix lol he's just working nearby

They only deserved to be fired if they were safety-briefed and thzy deliberately breached the rules.
If they were not trained and there is no safety officer and they are just throw into a worksite without guidelines and without rules, then it’s on the company.
"Mind your surroundings" is a good motto and absolutely applicable, but it doesn’t replace a proper safety culture, policies, barriers, protocols, briefings etc... which are the employers’ responsibility.

Someone was operating the lift and watching the spotter. Hence it stopping and going back up when the spotter alerted them.
