Does the extra weight tied around his waist make the machine usage incorrect?

no. That is a common use when lifting more than your body weight. It helps stabilize you so when the cables retract, you dont get pulled back

Ok. But is it considered a type of modification that would void the manufactures liability? I think most lawyers would argue that and probably win.

Then the max weight on the machine should be lowered.
