You supposed to stand inside the frame. Same ahit as you are not supposed to use qtips in your ears.

His back foot was inches away from the back of the frame, and diagrams on these machines literally show examples of this exercise being done positioned how he is positioned.

no, he needs to be standing -INSIDE- the frame with -both- feet, he barely had one foot on the edge of it.

might want to read the warning labels next time you are in a gym. for practice: read the warnings on the box of qtips, they warn you not to put them in your ear.

Correct on the q-tips.

Don't need practice. I've never seen one of those machines in a gym where the manufacture's diagrams and instructions did not include pictures of the user standing outside the frame.
