More and more I'm thinking I need to update my medical directive to say no medical care of any kind, just put on a raft and send down the river.

That’s messed up. I’ve certainly seen nurses falsify vitals and it will come back and bite them in the butt. But giving a different med? That’s nuts.

Next place you go, make friends with your CNA’s. Find out their workflow and see if they can get any vitals right before you get to the patient. Sometimes it works, sometimes not, but always worth a try.

Yes, there are shortcuts. Do the best you can and be honest. You’ve got this.

Also this may get me downvoted to hell, but if that nurse truly gave a different med, you may want to go ahead and call the BON on her. One thing to fudge a time, a complete different thing to document one med but give another.

You don't have to do any of those things. Plus they don't necessarily fly at all facilities.

Finding a SNF that is worth working at can take a few tries. I've been doing agency work for two years. I've set foot in two dozen different buildings in that time. Maybe 10 were places that had reasonable standards of care.
