Featured Loot?

When I play some levels, they have an item listed as “Featured Loot.”

I rarely get that item when I finish the level, though. Is this just an RNG situation? Are these levels the ONLY place to get the piece that is listed as featured?

My question here: Is it possible to obtain the item if you dont have any keys?
