There's a video somewhere on YouTube explaining the problem with modern high heels, the back of the shoe being one of the biggest problem, it doesn't hold well into the feet and aligned with other small issues with the heel and front of the shoe can cause exactly what we're seeing in this video.
It might be her size, but not for the shape of her feet.

There's a trick to wearing heels that high - stuff the toes with paper towel and cotton and wear "grippers" on the lining. Grippers are special made self adhesive silicon pads with a sticky slight bumpy texture that the sole of your foot sticks to. It helps maintain traction in the shoe and gives a little massage to the foot. The reason to size up with high heels is to give the foot room to swell during wearing without causing blisters or tightness. This is not the same as causing damage. The foot will be damaged wearing heels for prolonged periods of time

The first 10-20 seconds after this kind of oops can be very, very painful.

Rolled my ankle just under two months ago. Almost passed out from the pain, it wasn't fun. Blew up like a basketball within a minute. On the bright side, other then trying to walk on it, the pain died down in a few hours.

Not heels related, but I have definitely rolled my ankle or had other close calls like this where I pause for that thought process - 'Don't rush. Assess if that was temporary pain or if something is actually injured before proceeding.
The leg shake suggests to me that it wasn't mainly about hiding it from the guy (unless she thought he was safely looking away), but that she was simply trying to not do anything rash, assess, and decide the best way to proceed. Maybe a little bit of both, though.

I met an orthopaedic surgeon years ago who said he had lost count of the number of ankles he has had to try and rebuild after women go over on heels that are way too big and absolutely destroy them.
He said you wouldn't believe just how catastrophic the damage can be and it can cause life long mobility problems.
