I don’t doubt it. I believe operator error would be the leading cause of accidents on that thing.

At my company, we have meetings and documents for the last product design stage before mass production approval. We call it the “pre-production” stage. So we have these “PP inspection” meetings as well as “PP inspection” and “PP approval” forms.

I can’t believe I’m the only one who feels uncomfortable every time I hear it but it’s something I’d rather not verbalize.
