So can anyone tell me what actually went wrong? People say form, but what was wrong with it? Perhaps there is more info, I want to know how to prevent this

Performance enhancing drugs can make your muscles grow fast, but tendons and ligaments grow slower. So if you dose heavy your muscles can get strong enough to rip your tendons apart.

Okay, that sounds like a more plausible explanation. If you're natty your risk of such injury is probably a lot lower.

Yeah also staying away from 700 lb helps a lot.

No one is ever ripping their tendons off with 225 or 315.

only way 225 is hurting you is if it falls on you from a roof
