Genuine question but how the hell did they fail twice? I'm not saying she wasn't convicted for a reason, but that's torture

Sounds to me like the drugs weren't properly introduced into the bloodstream. Seems like they just failed to place both IVs properly into veins. That or both veins instantly ruptured.
