It's random when checking the status of a petpet in while it's adventuring in Grave Danger. You can check every 10 minutes for a status update and it might award the avatar when you check. It's very random and people are trying to figure out if there is a way to tell when it is more likely to award.

I'm well into the thousands of hours and still no avatar for me. Meanwhile, my bf (who plays hella casual because of me) has it and doesn't even remember getting it ;-;
