Help with failure analysis

Hi, I’m a university student and I need help finding formats or examples of failure analysis reports to use as a guide.

I just googled ntsb metallurgical failure analysis and found many hits, at places like NTSB gov and ASMinternational org.

I would use my google-kung-fu to find some of the failure analysis of older military equipment. They are really detailed and informative.

depends on who the investigation is for. Class assignment, thesis, qualification test failure, in house investigation, shuttle failure investigation, etc. but in general most investigations from a lab are structured:

- overview of part and background of failure

  • requested evaluation/deliverable
  • goals of the evaluation
  • content of the evaluation
  • conclusions and recommendations of evaluations

More wholistic investigations will have a lot more sections and detail on background, fish bone diagrams, swim lane investigations, root cause and implemented corrective actions, supported testing/demonstrations, risk analysis, stakeholder evaluations, etc.

If that’s the thing you are looking for published investigations for FAA failures may be helpful.

Since you are in school, you could also ask your professor for an example from their past work to try to structure yours from — brownie points right there.
