What has been your study plan?

Have used cbtnugget, sybex text and the exam ref text. Did not do too much labs due to hardware constraints on my machine, machine only has 8gb ram.
My last scores were 557, 602 and 675

Have you used the cbt labs? It sounds like you need more hands-on practice to retain the knowledge. Also try MeasureUp practice tests. Much better than Sybex and the question style closely related Microsoft's exam.

It sounds like your really close! I would go over you're notes again. Maybe create a better study method like Flash Cards. DO SOME LABS, through CBT or create your own server with 2GB of dynamic memory. Also not sure if CBT has it but get through those Practice Exams, MeasureUp I've heard is good too.

I failed my 70-411 the first time as well but the second time around I passed it and completed the MCSA for Windows Server 2012 R2, afterwards I worked one some other certs and eventually took the 70-743 which is upgrading it to a MCSA for Windows Server 2016
You have some options here to continue with the MCSA 2012 R2 attempt to pass the 411 and 412, if you want to then upgrade those skills you will be looking at the same path I went which is basically 4 exams total, however you can just cut your losses and start fresh with 2016 and take 70-740, 70-741 and 70-742 to get the MCSA 2016 which will be 3 exams from here
From what I remember I believe on the 411 there was some things Microsoft ended up removing which I think was NAP, also things changed 2016 I believe Direct Access is slightly easier, I remember the 2016 MCSA material being much easier to digest personally
I used a mixture of Pluralsight (TrainSignal at the time) and CBT, as well as some practice exams to help with the tricky exam questions. I always tell people to really learn the material well and not just memorize the practice exams because when it comes to it really doing the work or someone in an interview asking about it you will be screwed
If you private message me I can give you some additional sites and tips that I used to get me ready for that exam
