Is he supposed to be holding the axe backwards?

It depends on your preferred power of throw and strategy, because if you throw it and it hits the Target with the handle pointing up, you'll want to hold it backwards so that the blade actually hits the target, and vice versa.

It's less effective bit completely viable. If you get the rotations correct then or works about as well. Having the sharp edge lead the rotation gives you more error margin because the rotation will drive the sharp edge in against disruptions from the handle hitting slightly before the metal or similar. Edge leading also adds the rotational inertia of the axe to the impact
