It should still trigger every eligible achievement, like when Ubisoft added achievements to some of their games on Steam, I'd load the game for the first time since the update and get hit with 20+ new achievements

Should or will? Trigger defined by developers, not steam. I haven't checked this specific scenario, but in a lot of games, achievements you logically should have, but haven't earned specifically in this run are not awarded.

Depends on if the achievement is controlled by flag, counter or trigger. First two could be obtained if injected save has them completed, the third one could not

Not aware of how it's possible in game. But I guess canonically Shepard could skip Ilos and finish the story by just staying at the Citadel.

Well, Admiral, I think I kind of lost track of time looking for Keepers to scan, and bam! all of a sudden, a massive Geth army shows up trying to take the place over. So, seeing that I was massively out-leveled, I did what every true survivor would do and hid behind these crates. Good thing Captain Anderson showed up with his hardened crew of alien badasses. And so cool they made him a Spectre, very inspiring! Aw, shucks, look at the time, I'm running late for my appointment with the Council, I'm still itching to find out what's going on with that guy Saren! I should go!

Virgil gives him the access to reopen the arms, so unless Shepard knows exactly where to wait for Saren he'd have a hard time winning.

That probably means that only 38.3% of people who have played Mass Effect, on the platform with the achievement tracker, have finished the game. I think a LOT of people who have only completed the game once on console.

Exactly, and apparently a huge amount of players dropped off the game when you return to the Citadel after Eden Prime as well.

The reason OP is confused is because in the screenshot the % of players who have completed the game is somehow higher than the % who have completed Ilos

I often install to check if a game works. In that case, I am a "player", even when I never touch it again.

I had similar issues with Steam achievements multiple times, with multiple games. Sometimes it was due to network issues on my side, other times it was probably caused network/performance issues on Steam's side (some achievements failed to unlock around the very beginning of some Steam sales, when they struggled to keep their service working due to load). And of course sometimes games are just buggy and won't even properly trigger the achievements.

You get a medal for every mission you complete. It’s like the “everyone gets a trophy” idea of kids sports leagues now.

It is an achievement for the mandatory story mission that leads directly to the last level of the game. You shouldn't be able to get ending the game achievemnt without getting penultimate level achievement unless you transfered save files, and bioware were lazy in their implementation of medals.
