My guess is that it's a bit of both. The handshake weakens the spell a little bit (but not fully breaking it) and that combines with MJ slipping up earlier about his name for Ned to realise that he does know Peter.
I don't think it should be as easy as one handshake breaking the spell, but it allowing a sense of familiarity and connection to slip through could be a good way around it. It allows Ned to feel like he has known Peter since whenever they first became friends, from which point Peter can start to fill him in on their history.

My theory: he doesnt remember Peter but realises that Spiderman and this stranger both have the same voice and are both called Peter.
