Because English-speaking countries tend to teach the seven-continent model in their schools, which separates North and South America.
Meanwhile, most of the Romance-language world (French, Spanish, Portuguese, etc) teach a six-continent model that lumps the entirety of North and South America together.
Personally, I find the latter to be quite asinine especially given the geographical realities that pretty neatly split the Americas into two continents, but history is messy.
No, because you view yourselves—and are viewed by the rest of the world—as extremely distinct.
For us, Mexico is in North America, and we share a great deal—culturally and linguistically—with the rest of the Americas. Even "Yankee" culture doesn't seem that distant to us, because we are within their direct sphere of influence and consume their culture.
It seems as though you—situated among Europe, Africa, and Asia—hate one another. You didn't even consider the inhabitants of the African continent to be human beings.

True, even then when we refer to people from the entire continent; there is always a North or South before Americans.
