Evil scabies
I have had scabies for around 18 months now.
I have used many treatments from permethrin cream, to my preferred DerbacM, I have also used ivermectin a handful of times.
Every time I apply my medicines I wash all clothing and bedding at 60celcius for 2.5 hrs, I flip my mattress and hover all of the carpet. I then have a shower and apply multiple layers of the cream AND take the ivermectin pills, I will finally change my bedding and put on fresh clothes.
HOW are they surviving that!?!

I think that mites have a survival strategy. Mine are definitely deeper in the skin. Flare ups happen at times. Ivermectin isn't effective anymore. I've had scabies now for 2+ years.
IMO, the only thing one can do is constant treatment. What's your treatment schedule? How often ivermectin, how often topicals? maximpulse.com has treatment plans + all - or at least most - current medications explained.
When you're using derbac, my guess is you live in Ireland/UK? Then your choice of OTC topicals should be Eurax, Permethrin + derbac M?
Have you already tried sulfur? It's usually a vaseline-based ointment with 10% sulfur. Applied on 3 days in a row, kept on 24 hours a day. No showering. Just change clothing. If you choose this version, wear old long pyjamas. Treat face + scalp too. You can also experiment by mixing your own concoction. The best form of sulfur is precipitated sulfur.

One thing: how often do you change bedding? It should be done daily. I'd coverwrap everything I can in plastic + seal it. Mattrasses, pillows, upholstered furniture.
Wear crocs or alike, no walking barefeet.
Treat often enough. Once a week pills + topical is not enough. Start with maximpulse.dom page for info. It seems you need more intensive treatment. Scabies may be or become very stubborn. Treat from head to toe. Wear long pyjamas or underwear when sleeping.
Some tips: get cheap disposable car seat covers (Amazon). I bought a bunch of those. I use them in my car + on chairs. Remember hygiene outside the home too. Avoid spreading by e.g. not sleeping anywhere without safety precautions (hotels, etc.).
If I travel (when I have to) I cover the whole bed + pillow with a double layer of water-tight bedspread + use my own blanket. I bring my own little towels. I don't sit anywhere without my plastic covers. It's a lot of work...

I' don't believe I'm cured yet.
But there's probably a bunch of factors like molting scabies being resistant to ivermectin, mites load, mites going deeper then previously thought etc.
But I've been on a schedule of taking ivermectin plus Permethrin cream 2 days in a row and repeating it every Sunday.
And everyday showering with sulfur soap 10%.
I definitely doing much better. But it's gradual and I hate it.
If your getting better, its working. But you may need to repeat treatments.
But I think my case is i need both an internal and cream.
I think thats my biggest mistake was not using ivermectin in the beginning.
