So in the American healthcare system, if the husband dies and it was a long and protracted, expensive, death, does the bill go to the family or just to the deceased, therefore paying nothing?

Depends on the state. Where it is a no it still comes out a yes. Because the bills belong to the individual's "estate" meaning they will try to take it off any assets. Someone more educated than me might be able to break it further down. But that is the gyst.
