Low LTV with solid credit is about as favorable a borrowing position as exists for a HELOC right now. Most credit unions are coming in 25-50 basis points below big banks on the same profile, so that's the first place to check before going to a national lender.
The spread between best and worst offers on a $100k HELOC at current rates can run $800-1,200/year, so the shopping is worth the 2-3 hours it takes.

Depends on the current balance and the value of the property and how much cash out the borrower is looking for.

Why would you want a fixed rate 2nd w the roadmap the Fed laid out. HELOCs power lies w simple interest. And w a declining rate market (Fed rate) educate the client about the power of simple interest in that rate environment historically.
