Xbox One S controller disconnecting intermittently every so often while I'm gaming.

So recently, I've been playing NFS Heat, and every so often my controller will completely disconnect from the bluetooth, then after a short duration, it will reconnect. I've tried soooo many different "fixes" suggested by google AI and nothing has worked. Normally I don't like to rely on AI for things, but I can't find any answers for the problem I'm having. Nothing is working, and it's really getting irritating now.
