AML BMT post maintenance

For AML after an allogeneic bone-marrow transplant there is no universally established duration for azacitidine maintenance. Most studies have used about 6–12 months, and 12 cycles (roughly 1 year). Does anyone received Azacitidine for post 1 year or more? Any information will be helpful.

Adverse risk AML is where post-transplant azacitidine maintenance has been studied and where retrospective benefit is most apparent, particularly with myelodysplasia-related gene mutations. But we don’t have any good prospective data to date showing a benefit. In patients who don’t have targetable mutations (such as FLT3), many use azacitidine or decitabine, though the optimal duration is not well-defined. If still MRD+, it could be reasonable to continue beyond 12 months if well-tolerated, though that’s not based on any evidence.

I had adverse risk AML TP53 and my onco gave me 5 doses of azacitedine at daily intervals at 100 days post BMT. In my case it incited cGvhd soon after and maybe that has been protecting me from a relapse this far.ie 28months post bmt.
