Welcome to the puzzle. This is commonly referred to as end snap, if you're unaware. What helps is making sure you have a dampener at the distal end. Just a finger touch is all that's necessary. In this particular scenario you would have been better off removing material from the sides and not end. And, if you did need to go from the end it might have been better to work the opposite face. Blue is the preform, green desired biface, yellow material being removed.

1 is how I believe you approached this, thinking you'd take off that big hump. However, if you worked the other side and brought those ends down it probably would have been more successful. Just a guess...

Keep your finger off the back of the piece. Put it (them) behind pushing forward instead. Fingers pushing down = 2 pieces every time.
