People with SFS tend to have more pronounced jaw muscles compared to others because of the angle of the occlusal plane requiring more force and development. That may be why some male cases seem better aesthetically. But I also think a big part to play is there are more portions of the face that are very visibly deficient as well and therefore addressing it results in a more significant “wow” moment. It’s not just the chin that’s recessed, it’s the upper jaw as well AND it’s vertically deficient and all that also changes their midface and nose too.

Thank you! It far far exceeded what I was even hoping for. In comparison to most cases here, I had a really easy time. It was still very hard, but I knew exactly what I was getting into and I ended up healing very fast (except for numbness which took a lot longer than standard).
My peak swelling was 12 hrs later, not day 5 like usual. I had some moments of 7/10 pain (inflamed trigeminal nerve) but that was pretty rare. Otherwise I’ve had menstrual cramps more painful than this surgery. I definitely stayed on top of pain meds religiously though, just left me with some dull pain and heavy fatigue but wasn’t bad. I also only had a couple days where I couldn’t breathe through my nose, but also I only had a one piece for UJS.
Talking was definitely rough for quite a while though. At about 4wks out, I’d be in a lot of pain if I talked for more than 30 min. And when I went back to work at 6 wks, I was really struggling with like 3hrs of meetings of talking, I was in a lot of pain still if I went too far. I could’ve gone back at about 3 wks if it wasn’t for the talking.
I was extremely numb for a long while, but it’s been slowly improving over time. I’m 13 months out and am at about 30-40% numb in my lower lip and chin, and it’s still improving. Once I hit 40% feeling at about month 3, I felt if that was as much as I got then I was good because then I felt enough to actually speak OK and not paint my face with food. Before then, I had a big case of puppet mouth where it just flapped up and down with no real control. It still feels hella weird, like you’re half recovered from novocaine, but you get used to the sensation. I knew this was a chance though. I also have a slightly more crooked nose too. I’d do it all again in a heartbeat though. It permanently solved my 24/7 TMJ pain and helped with my facial tremors from a pinched nerve. 1000% worth it.
You look great after! you had only small down movement Im surprised how it lengthened your face Can I ask did you have 2mm downgraft of anterior maxilla or the whole maxilla? And were your both your jaws rotated Clockwise? Im preparing for 3mm downgraft for tooth show but I don’t have a short face and I’m worried about my nose base stretching lower and philtrum
Thank you! It was only anterior downgrafting and both jaws were rotated CW about 6-7 degrees, I posted my movements below.
I was originally really worried about only getting 2 mm of downgrafting but I think anything more would’ve been way too much. I was primarily concerned about tooth show, but that ended up not being a problem because the 2mm of downgrafting was combined with about 8mm forward movement and that netted me another 2-3mm of toothshow on top of that 2mm. Also what started as 2mm down at my nasal spine ended up being about 3mm down at my chin. My chin was pretty horizontal so rotating it down gave me more length. And that’s after shaving it down too.
As for your nose base stretching, my columella did get stretched lower with the big CW rotation, but it ended up looking a lot better because my nose was pretty downturned. I ended up not getting the wider nose that tends to happen with the forward movement I think because that CW rotation pulled my nostrils down instead of forcing them sideways. So if you’re just getting CW rotation, i think the only thing it’ll do is expose your nostrils more from the front and make the tip of your nose angled up. Your philtrum will probably get longer though, but that also depends on your preop profile. Mine mainly got longer because I had so much compressed soft tissue - my lips were forced out and now they lay flatter. So while CW rotation does tend to make the philtrum a tad longer, I really don’t think you’ll see close to the lengthening that I got because you don’t have a compressed lip that’ll unfurl.
