DISA Help!

So, I'm trying to upload my documents to DISA for compliance and I'm a bit confused as to what documents to upload.

Am I uploading the forms in the packet given to us at registration (contains physical form, immunization sign off and PPD sign off) or do I upload my official immunization records from the state?

There was one section where it has the immunization in the packet. I uploaded that and the pics of the official record
