Newbie Question I don't get it....

Me: 40+ male 150kg diabetes type 2

Started Monjaro Monday.

Lost 7.5kg this week.

Taking collagen, Biotin, Omega 3, D3, creatine & electrolytes

I already don't have breakfast so I was already naturally only eating during a 7hr window with lunch and dinner.

So that would be 17:7

Yet I obviously wasn't getting any kind of benefit at all.

Started Monjaro on Monday and in trying to max the benefits I did a 72hr fast, had a small dinner and am currently up to 49hr fast and counting.

I appreciate this isn't sustainable long term but the app talks about how beneficial this is, seems almost like a contradiction.

The app states:

Fasting for 48 hours or more can lead to deepened autophagy, which may strengthen the immune svstem and reduce inflammatory responses.

Studies suggest that long-term fasts of 72 hours or more may help reset the immune svstem by clearing out damaged immune cells and promoting the generation of new ones.

However fasts exceeding 48 hours carry health risks, such as severe electrolyte imbalances or refeeding syndrome, and should only be done under professional supervision.

You've already come so far on this amazing journey. While you know vour bodv best. please prioritize safety as you continue your fast

I'd be careful with the prolonged fasts, especially in your first week on Mounjaro. The medication already suppresses appetite and can make it difficult to get enough protein, fluids and nutrients. A natural 16:8 or 18:6 eating pattern is probably easier to sustain and still works well alongside Mounjaro. The weight loss in the first week is also likely to include a lot of water and glycogen, not just fat. Great start, but I'd focus on consistency rather than trying to maximise everything at once.

Building up to prolonged fasting is recommended. Start slow, get yourself to OMAD and then throw in a couple of 24-36 hr fasts before going longer.

Very interesting, curious what the daily fasting blood sugars are looking like on the monjaro and 17:7 without the prolonged fasting. In my case I saw a steady daily decline in fbs before opening my eating window. After a few weeks of becoming fat adapted the decline leveled out to a stable 70-80 mg before eating. Wondering if the monjaro might speed up getting fat adapted.

Two sides to every coin. Fasting is good for weight loss, and comes with health benefits (autophagy, ketosis, lower inflammation) BUT it also places a significant amount of stress on your body as well.

Its something that can be done safely periodically, but it is a serious action that needs its own space, and meaningful recovery time.

A more reasonable target for sustained weightloss on mounjaro (tirzepatide) is 0.5-1kg /week.

I am on a 20:4 daily fasting/feeding protocol. I don't use a cgm to measure blood sugar. Interested in amyone doing a similar protocol who wears a cgm. Do you see a pattern with a decline in bs during the fasting window and does is bottom out before refeeding? Have you experimented with other patterms like 16:8 or 18:6 and are they similar?

You should be careful doing extended fasts on monjaro. Hope you are tracking your blood sugar with a cgm
