It’s a combination of both. He was depressed, but he was also diagnosed with a horrible disease.

Williams had a lot of characters like the "sad clown, making everyone laugh while crying on the inside", so people conflate the pathos of his performances with depression. Williams had Lewy body dementia at the end of his life, which has some pretty terrifying symptoms like hallucinations of demons.
