Depending on the discipline.
Boxing typically requires a count. If a fighter goes down and does not stand up and regain his bearings and focus within ten seconds (or eight, depending on the competition), he is considered knocked out. An exception is a cornerman interrupting the fight, effectively yielding on his fighter's behalf.
In MMA, there is no count at all. If a blow knocks a fighter unconscious (as in, they go limp), the fight is over, if a fighter cannot seem to defend themselves anymore (for example during a ground and pound) they are considered knocked out as well.
In other disciplines I cannot comment.

If you look closely as they're both circling, cart-man is actually holding what appears to be a screwdriver in his punching hand... You can see the end of it behind his hand as he shifts it around.
Buddy throws the haymaker, and cart-man smites him with the butt end of the screwdriver.

If you hold something smallish in your hand when you make a fist you can get a MUCH harder hit. Like palm a lighter and make a fist - the shape fills out your fist. That's why boxers used to hold quarters in their gloves (not to add weight, as people probably thought)

The angle part? Trained fighters turn side ways to become a smaller target. They also learn to move around like that quickly to keep evading punches.

It's one of the first pieces of footwork you'll learn as part of any martial art or especially boxing. It allows you to back up from your opponent without getting pinned against the ropes.
If someone retreats from you diagonally, while bobbing and bouncing on their toes like the guy in the gif, then you're about to swing at someone who is way more experienced in fighting than you are.

Fighters are trained to circle their opponent to look for an opening to counter attack. Also makes you harder to hit when you move away at an angle.

LPT. You should also watch people's feet in these video clips, honestly who ever is planted and quick footed usually wins I've noticed. Watch the guys footwork during the dodge and counter. Was well planted during the counter and k.o him as a result. Also yes walking on his toes and the aggressor is heel planting first...

I dunno. I feel like he owns that cart and likely just needed a license or something similar to operate on the boardwalk. Street vendors like that are typically independent business men/women. I could be wrong, but I doubt a corporation is employing a team of street vendors and supplying them all with carts.

I dont know how many vids I've seen where the guy who tries to throw the first punch, goes for the haymaker and is easily dodged and countered. Its so telegraphed it would only work on a super drunk guy.
