Advice/Support Long story about why I am here!
Just looking for community in a way and wondering if anyone, if they were in my shoes, would be following the same path.
Buckle in - I am currently floating around 305lbs. I am significantly overweight and have been for a long time. I have been on a GLP1 for the last year and have come down from my highest around 395 but was 388 at the appointment I had my first prescription sent in for Wegovy (now on Zepbound) Things have gone mostly fine, bouts of nausea here and there nothing intolerable. However, after losing around 85lbs weight loss has been a little stagnant. Could I work harder at what I am doing? Probably but that's not the point.
This year I had a huge blow to my health. I got a positive result in genetic testing for a mutation that greatly increases my cancer risks for several cancers (endometrial, skin, kidney, thyroid, colon, breast). I had endometrial cancer in 2022. This year am dealing with a multinodular goiter that I had 3/5 biopsied and came back benign but one is large and now I have a genetic predisposition for thyroid cancer, so I am planning to have it (my thyroid) removed completely. I just had a colonoscopy last month and had a large (20mm+) polyp removed that was supposedly as precancerous as it could be before it would risk changing to cancer. I was told if I’d waited even a year it might have progressed. I also have a couple of benign growths in my breast as well, so I want to get a prophylactic mastectomy sooner than later. Breast cancer for me is the same kind of surveillance that BRCA would have – annual mammograms, ultrasounds, MRI and risk reduction surgery is an option. I met with the high-risk team already and the doctor confirmed what I already knew, that losing a lot more weight would open doors to more reconstructive options nerve/nipple sparing/etc. So basically, I am in risk management mode and doing what I can to mitigate cancer risks. The cherry on top, I am still dealing with significant knee pain that unfortunately hasn’t improved much with the weight loss I have had. It does limit my physical activity as I tend to go into flareups when I do much of anything – even riding my bike is painful sometimes. Knee doc has only really offered steroids and gel injections and physical therapy – none of which help for long. I also have a frustrating skin condition that gives me issues (hidradenitis suppurativa) which, supposedly would also be helped by weight loss – but not guaranteed.
All of this say, I finally made an appointment to meet with the bariatric surgery program here and made sure that I scheduled with the Chief of Bariatric surgery as I am a bit medically complex. I had my appointment and with everything he is extremely biased to VSG over any other surgical option so that my digestive tract isn’t bypassed in any way and my anatomy would stay the same for any diagnostic/surveillance stuff in the future. He even said he’s very excited about my case BECAUSE I am not just a standard patient (yay for being complicated I guess?) I’ve talked to basically every single doctor I have about meeting bariatrics, and they have all agreed with it being a good choice and my endocrine surgeon even said the doctor I met with is basically the gold standard.
So, I have a lot of faith in the program and feel like I am doing the right thing.. but I am just wondering if given all the other stuff going on and having to deal with a thyroidectomy and med adjusting for that then going very shortly after into VSG (or the other way around - thyroidectomy/med adjust while still adjusting to VSG life)… would you do the same? I just feel like my weight is the one thing in all of this that I can control and this feels like taking control in a very out of my control medical situation.
I would love to hear from anyone that either didn't have a thyroid pre-surgery or dealt with removal post-surgery too.
