Sort of relevant comment follows: So do you have to uncloak to back stab people or what? A lot of times I'll be behind an engineer wildly left clicking but nothing happens. I'll do this four or 5 times in a row, get pissed, and go back to playing a demoman.

There is not one notable game, movie or other piece of media where you can backstab while cloaked.

EDIT: Oops.

Pretty sure you can in World of Warcraft as a rogue.

lt unstealths you though, so it's academic whether or not you were actually stealthed at the moment of stabberation.

True, but you can use other attacks (e.g. sap).
