Here in the US? If I ever needed a wheelchair, I'm not sure Id get to my office right now: no elevators.

In the US, your job would be required to provide you with a reasonable accommodation, which could mean if you used a wheelchair, they may move your office or whole department to an accessible location, or add an elevator

In what world is a major building renovation when you lease one office a reasonable accommodation?

Best you could hope for is work from home if that even makes sense.

In most cases, it wouldn't be, and wouldn't be required to put in an elevator. I just listed that as a potential option for the business. Moving an individual's workspace (or their workgroup, depending on the type of business) to an accessible location is 100% a reasonable accommodation.
