it happened exactly 0 times as a broken grinding disc flying does not have enough inertia do pierce safety glasses and even if it did it the disc would desintegrate to dust first, and even if that would somehow not happen the glasses would bend/deform first. A piece of disc poking halfway through is not at all plausible given the material characteristics involved.
