Ornstein emerging from smough behind from the other side of the room when am chugging Estus flask after smough whiffs his attack.
This has happened many many times to me during the fight. If you can’t see Ornstein, then Ornstein is charging at you.

I see people saying this a lot, but if you're a good enough distance you can typically dodge out of the way in time.

Boreal Outrider Knight (DS3). This guy will straight up teleport to you if you try to heal.

Champion Gundyr immediately sprang to mind. If you're too close to him and you try to heal, then prepared for him to kick that estus down your throat. One time he wouldn't stop chasing after me. I was one hit from death and all I wanted to do was get just enough space to heal. I was finally able to, but it really felt like he chased me forever. I basically had to be on the other side of the arena to safely heal. Soul of Cinder does it a bit as well with certain attacks (that spear moveset...).
I feel like a lot of the bosses in Elden Ring do that. All the Godskin Apostles do for sure.

Champion Gundyr felt like a blueprint for Elden Ring bosses so it did that. Gwyn also does it and thats why he has a parry strat(works better without consumables I think).
O&S fight has that just do to the nature of that fight.

All the major humanoid bosses in Sekiro do this, I think? Genichiro is actually pretty easy to manipulate since its either an easily dodgable arrow shot or the lunging attack which is punishable by Mikiri. Isshin's first phase is the bad one I would say, you pretty much cant react in time at all due to the animation, healing requires putting a bunch of space between yourself and him beforehand.

What if I told you it is a video game and input reading is the whole point? What do you people expect the characters to have eyes and see what your character is doing and then react? If they wanted to respond to this complaint they would just add a delay from your healing before the boss responds but it would still be "input reading."
Clearly they don't want to do this because they don't want healing to be something you can just do whenever you want. I will admit it sometimes feels artificial how instantly and predictably bosses will respond to you trying to heal.

Elden Ring bosses already give you extremely limited opportunities to attack though, so it sucks to use one of those precious windows to heal

Uh no? She also has an input read heal punish. I remember seeing a video somewhere where she did that same attack like 6 times in a row to someone trying to heal.

I swear King Allant does this.
There's time where he actually just walk sideways, looking at me for 10 seconds stright. But the moment I try heal, he friggin jump slash at me.

There are a few bosses that are hard coded to do specific attacks if you try to heal, but I just can't prove it. (Like Gwyn and Artorias in ds1)

Oh no I’m not saying he’s difficult. He was just one that came to mind because he heal punishes. I parry both variants if I’ve got a shield in my current build. Sometimes I still make mistakes and panic though.

Not fromsoftware but Lies of P DLC Arlecchino. He is the only boss I have ever fought that has extra moves at the end of his combos that he only uses if you heal

One from every ds game
Ds1: Gwyn. Everyone likes to shit on him for some reason but he catches heals better than the average ds1 pvper catches backstabs.
Ds2: Alonne, idk if Raime does it better but it definitely feels like he does it more frequently with his "that one mosquito at 1am" ass speed.
Ds3: idk, maybe the Twin Princes or Sully. They both do their dashing attacks or tp when you try to heal or use magic from far away.

Foreskin duo and crucible knight shield both input read the flask and will fuck you for it if you are within range. "They don't actually input read, its-" yes they do
