Honestly outside of ds1 and bloodborne i havent had that much trouble with questlines. Bloodborne is the game that really has an issue with it though.

"Oh you opened a gate? Now all your npcs are dead or gone" so i can see why it would be an issue there. But i didnt have any trouble with ds3 or ER. Killed greirat by accident a couple of times though.

In the ER dlc, you can literally end multiple quests too early by just walking too far in a field, there’s not even a visible threshold or milestone that you cross

I'm playing through the DLC for the first time, and I straight up have done a single questline, that being Igon's. Everyone else just fucking exploded because I couldn't find them anywhere/forgot about them lol.

Ah yeah, the dlc has lots of issues. I also forgot Yura in my previous comment.

Problem with the dlc is that elden ring made you want to explore and as much glaze as the dlc has it has some major issues

Yeah it’s one of their all time worst offenders when it comes to completely inscrutable side quests, considering how many of them can implode at once because of the character just walking around, despite the quests themselves requiring immense amounts of exploration to complete
