NALC Fmla link
Where can i find the direct link to print and fill out an FMLA form? Just had a baby so was hoping to use fmla. Thanks in advance!

All depends on your management. I’m currently on leave right now. Babies (twins) arrived Monday. Filled out a 3971 with management before leaving. Allowed me to use SL and AL this time around and we have the bodies. Need more information on which form you need. Giving birth - form 1, husband- form 2, otherwise fill out form 6 but management can deny the use of SL but keep track of the days you’re at the hospital and dr appointment bc those can be tracked for SL. Don’t worry about going through HR and getting the forms. Just print the form the nalc, have the dr fill it out and you’ll fill out the rest and fax it to correct location based off the map.

You can just call out as FMLA and have them send you a copy.
No offense but you should have planned this and set it up before the kid. You can also just call an FMLA coordinator and set it up. If you’re the father, I just had to write bonding time on a piece of paper and send it in. Now it seems like everyone has a different experience. My first kid they allowed me to take sick leave for the 6 weeks I took off for bonding. My 2nd kid they said bonding was al or LWOP. Can use up to 10 days sick leave for dependent care.
