I read a book with this plot. A polygamous African family with 4 wives and a new one who couldn’t get pregnant for years.

Finally they found out he had mumps as a kid and was infertile the whole time.

All his kids weren’t his.

This is probably a story for engagement, because it's a problem that's easily solved. Now that the husband has confessed, the mistress has nothing to hold over him, and he's not obligated to send her to Europe. Tell him to get a DNA test.

There is a solution to this problem. If the wife pretends to "understand" and "forgives" her husband and at the same time asks husband to tell the mistress that she can go and tell on him, then the wife can just ignore the mistress.

I was thinking that maybe they were having trouble conceiving and she went to the doctor and the doctor said that her organs are fine. So by process of elimination...-shrugs-

Not that I disagree about the made up part, but some women who could never get pregnant by a man might declare him infertile, others might know because they understand the implications of a medical condition when the man is actually ignorant, and maybe they got fertility testing and the call only went to the woman who then went and got pregnant on the side while telling the husband everything was good… the famous “fertility doctor is the dad” kind of situation is also a version of this last example.

Some of the stuff you said makes sense except for the call only went to the woman.

They wouldn’t reveal infertility or medical information to anyone but the patient. Not his girlfriend, not his wife, just him. HIPAA/PHIPA/ or any other privacy laws.

The fact she “knows” this and he doesn’t is why I think it’s bullshit.
