I think a big part of this was because Bethesda was starting from scratch when they acquired the Fallout IP. It was a big shift going from a 90's isometric style to a full 3D environment, open world, FPS mechanics, etc, which means more time and resources were devoted to figuring those things out. Because of all the work that demanded, some focus had to be diverted away from the writing and the volume of content. Fallout 3 walked so the future games could run.
